Our Clinical Team & Services

Our Model

Welcome! We offer a healthcare approach that doesn’t just treat symptoms or diagnoses, but supports you in rewriting your own health story.

Our practice is built on a transformative model of care where the relationship between clinician and patient is recognized as a source of healing for both individuals, and where the healing generated in this dynamic has a

profoundly positive effect on our communities.

We offer a multidisciplinary collaborative clinical practice with medical and psychiatric services, we ensure that our patients benefit from the diverse expertise of a full care team working in unison. This collaborative environment not only enhances the quality of care but also ensures a more comprehensive and

supportive experience for every individual we partner with.

We operate from a foundation of shared humanity, understanding that the health of the individual is inextricably linked to the health of the broader community.

By consciously divesting from corporate medical structures, we center both the clinicians and those receiving care as equals. This approach fosters a collaborative environment that values clinical expertise and lived experience equally, ultimately serving the greater good and contributing to the collective liberation of all.

We provide a collaborative care model that supports mental and physical health and wellbeing. Our treatment plans are always comprehensive, and include lifestyle and integrated recommendations tailored to individual needs and preferences.

I am currently in-network with BCBS of Vermont for Primary Care and Specialty Medical with Follow up, MVP pending. All other services are cash pay.

Hi! I’m Grace Altebrando, FNP-BC, IFMCP, MSCP. I am a Family Nurse Practitioner here in Vermont. I offer primary care and specialty medical care for peri/menopause, postpartum support,

and overall lifestyle interventions for improved vitality. â€‹I studied Psychology at

McGill University before training as aNurse Practitioner at Columbia University.

My approach to primary care treats the whole person—physical, mental, emotional, social, and environmental—rather than focusing only on isolated symptoms or diseases. I combine evidence-based conventional medicine with validated lifestyle, behavioral, and complementary strategies (nutrition, exercise, sleep, stress management, mind–body practices, prudent use of herbs and supplements) to prevent illness, manage chronic conditions, and optimize overall well‑being. Core features include continuity of care, shared decision‑making, attention to social determinants of health, personalized prevention and self‑management support.

I am a Menopause Society Certified Practitioner who supports women through the menopausal transition. I combine evidence‑based medical knowledge with skills in hormonal and nonhormonal symptom management, preventive care, lifestyle counseling, and psychosocial support to help people navigate perimenopause, menopause, and beyond.

I am a Perinatal Mental Health trained practitioner who works to support the mental and physical health of parents during the postpartum period. I focus on physical recovery, mental-emotional wellbeing, lactation, and transition to parenthood.

I also offer Reiki and sound healing services. Reiki is a form of energy healing in which a practitioner places their hands lightly on or just above a recipient’s body to promote relaxation and well‑being. It is based on the idea of channeling energy to support the body’s natural healing. Sessions can be offered with or without sound healing with tuning forks. Sound healing with tuning forks is a complementary healing practice using calibrated tuning forks applied near or on the body, held over chakras/meridian points, or used in the surrounding space to produce sound and vibration intended to promote relaxation, balance, and energetic alignment.

I enjoy time in my garden, wildcrafting, hiking with my dog, reading, yoga, meditation, various art mediums, and my wonderful family and friends.

Our Mission

We believe that in order to create a truly integrated model for healing that we, as clinicians, need to take back ownership of medicine.  Corporate medicine as it stands is a broken system, with health care disparities for marginalized individuals.  This system inherently erodes the relationship between patient and clinician.   Having an independently owned and operated medical practice enables us to spend the time and energy we need in order to create healing relationships with our patients and ourselves.  We do advocate at the national level for mental health parity laws, improved accessibility to healthcare, and an overall more just system.

Partners In Integrative Healing is currently in-network with Blue Cross Blue Shield of Vermont.

We are in the process of credentialing with MVP and are accepting patients for the waiting list.

We are out-of network with all other major insurance plans, most insurance plans

will offer out-of-network payments, we can provide a superbill for services rendered

and submit to your insurance plan for you. We also offer cash-pay options for our clinical services.

Interventional Services

Our affiliated practice, State Street TMS, PLLC offers interventional approaches for the treatment

of  major depression with  transcranial magnetic stimulation (TMS) through

multiple insurance plans including Vermont Medicare and Medicaid.

We do offer both standard and customized accelerated protocols for depression that are based on the SAINT protocol, as well as customized protocols for OCD, anxiety, ADHD and PTSD.
